1968 Chrysler NY fender "tags tag"

There is a little sheet metal tag under the main tag , sitting with one of the two screws.
What is this one telling you wiser people. I have had my car for about three weeks now and everything is decoded except for this.
On my car there is a 5 stamped from the backside , not sure if there is anything else on it.
