Looks like a dropped valve, but when that happens there is usually a lot more damage to the cylinder wall and the piston. Well. . . he for sure wasn't using 2-cycle marine oil with TCW-3. . .

That would be some mighty nasty detonation, punching holes.through pistons and what not.detonation or pre-ignition would be my guess
