A couple more whoops from work

This came from a 3.5L in a Colorado. I had a little knock as the piston was hitting the crank.
 
How the hell Does that happen?
It had to have been a fuel hydro lock, it came in the shop running good but would quit after about 20 minutes. We found a bad PCM, we sent it and a replacement PCM to the dealer to be reflashed to the truck. They couldn't get it to flash to the truck, said there was already a VIN # in the PCM. We sent another PCM to them and they flashed it to the truck and the tech says when he started it the engine had the knock. We believe the first PCM was defective and loaded the cylinder with fuel and when the engine took off it bent the rod.

The thing the really sucks is the company the computer came from won't do anything about it.
